How to Complete Your Monthly Income & Expense Statement

  • All reports are filed for the calendar month. For example, for your January report you will include all pay stubs where the pay date was in January. Reports are based on when you got paid, not when you earned the money.
  • All reports are due by the 10th of the following month. For example, your January report must be in our office no later than February 10.
  • Photocopies of pay stubs and other information are acceptable; we do not require originals. In fact, all documents you send us will be scanned and then shredded. So, if you want to keep your originals, please send us copies.
  • We strongly recommend that you either:
    • fax your reports to our office at 1-877-347-0660, or
    • scan and e-mail them to statements@hoyes.com, so that you know they are received on time, and so that you have a copy for your records, or
    • mail them to Head Office at 2A-800 King Street West, Kitchener, Ontario N2G 1E8.

If you don’t have access to a scanner, there are phone apps that you can use to take a picture of your documents, convert them to a pdf file, and then email them to us.  Our clients have used the following apps:

  • Genius Scan for both Apple and Android (basic app is free, extra features like cloud storage cost extra)
  • iScanner, for both Apple and Android (the app has a one time cost of around $7)
  • CamScanner, for both Apple and Android, basic account is free
  • Tiny Scanner for Apple or Android, basic account is free.

If you are emailing documents to us, pdf format is recommended, as picture formats, like jpg, sometimes are too large to be transmitted.

To ensure that your bankruptcy finishes as soon as possible, please attach the following to your monthly report:

Proof of Income

  • All pay stubs for the month for all family members;
  • All stubs or copies of cheques for other sources of income (support payments, pensions, Child Tax Benefit, Universal Child Care Notice, Employment Insurance, Disability, Social Assistance, etc.);
  • If your payment is electronically deposited and you do not receive a stub, please attach a copy of your bank statement, or a print out from the bank machine
  • If you are self-employed, we require a Summary of Self Employment Income, including receipts for any tax installments paid.

Proof of Expenses

We do not require receipts for normal monthly expenses (rent, groceries, etc.). We do require receipts for the following expenses:

-Child and spousal support paid (including a copy of the support agreement for our files);

-Day care;

-Prescription drugs and other medical expenses;

-Court ordered fines (such as speeding tickets);

-Tax deductible employment expenses (provide copy of T2200)
